Registration Step Process

FAQ

(please read before proceeding)

  • Do you have an urgent need, and you’re not yet a patient?
  • We can schedule you as an urgent care visit If your medical records have not yet been received. However, on your first new patient visit we waive the new patient fee.


  • Telemedicine?
  • We currently DO NOT provide telemedicine as we believe care is best handled in person.


  • What about insurance?
  • Heritage has never participated with any commercial insurers. Most insurances allow patients to self‐submit their own claims—the way the process originally worked. We provide a standard insurance form with each patient encounter which can be submitted by the patient to their insurance for reimbursement usually at an out-of-network rate. We DO NOT take Medicaid or Gateway.


  • How about Medicare (CMS)?
  • We can still see Medicare patients. However, according to §40 of the Medicare Benefit Policy Manual, we have “opted‐out” of Medicare and notified the CMS carrier via affidavit. Patients will need to sign a contract with us stating that neither Heritage Family Health nor the patient will submit a claim on behalf of services provided at our office. Medicare benefits will still cover labs, images, etc. that we order for the patient. A copy of this affidavit as well as the private contract sample is available upon request.


  • What about workman’s compensation or motor vehicle accident?
  • In the case of wanting to become a patient while under workman’s compensation, please call 717-949-4811. We DO NOT take new patients seeking care while under a motor vehicle accident claim. 

Please follow the steps below to become a patient at Heritage Family Health.

(Note: completion of the following STEPS could take 4-6 weeks.)

STEP ONE: AUTHORIZATION FOR DISCLOSURE OF HEALTH INFORMATION 

You can fill out a transfer of medical records form at your current doctor’s office OR you can come to Heritage to fill out our form and send it to your current office.

  • This form goes to your current office with the goal of having them send your records to Heritage Family Health.
  • You must do this per individual family member; 18+ will have to fill out their own.

STEP TWO:  PRE-REGRISTRATION INFORMATION

  • This will give us all the information we need to start to build your patient chart.

STEP THREE: ACKNOWLEDGEMENT FORM (Complete now or first day in clinic)

  • Make sure ALL sections of this form are filled in and signed.
  • CONSENT FOR DISCLOSURE: make sure you include ALL parties who can obtain your information. This cannot be assumed.

WHAT TO EXPECT:

When you have completed both the registration form and sent your medical records from your prior office, we will add them to your chart here at Heritage Family Health. After this you will receive a phone call to schedule your first get-established visit. This visit should be scheduled as soon as possible; however, is required within the first six (6) months!
